Shoulder Pain from Gym, Pickleball & Cricket: Physiotherapy Treatment Guide

That pinch in your shoulder after a heavy bench-press day, a long pickleball session or Sunday box cricket is not something to "sleep off". Sports-related shoulder pain is a growing reason active adults in Ahmedabad book a physiotherapy assessment. The good news: most of it responds well to a structured, non-surgical rehab plan when caught early.

Why Overhead Shoulder Pain Is Rising in 2026
The shoulder is the most mobile joint in the body, held stable by four small rotator cuff muscles. Fitness habits in Indian cities have shifted fast: more people lift weights, box cricket turfs are full every weekend, and pickleball courts have opened across Ahmedabad. Doctors have reported that this boom has brought a rise in pickleball-related injuries, with the shoulder among the commonly affected joints.
Most players are desk workers from Monday to Friday. Rounded shoulders and a stiff upper back narrow the space under the shoulder bone (acromion). Then, on the weekend, that same shoulder is asked to smash, throw or press heavy weight.
The most common triggers we see:
- Sudden jump in playing time: going from zero to daily pickleball or cricket in a few weeks.
- Ego lifting: heavy overhead press, bench press or dips with poor form.
- No warm-up: walking straight from the car park to the court.
- Desk posture: forward-rounded shoulders that pinch tendons during reaching.
- Playing through the small ache: ignoring mild pain for weeks until it limits sleep or lifting.
Common Sports Shoulder Problems at a Glance
Not every sore shoulder is a tear. Most sports shoulder pain falls into a few recognisable patterns.
Six Patterns We See Most Often
How each one feels, and what rehab focuses on
Subacromial impingement
Pinching when lifting the arm between shoulder height and overhead, usually from smashes or shoulder press. Rehab focuses on scapular control and posterior capsule mobility.
Rotator cuff tendinopathy
A dull ache on the outer arm, night pain and weak lifting after repeated serves or high-volume gym sessions. Rehab uses progressive rotator cuff loading.
Biceps tendon irritation
Pain at the front of the shoulder, worse reaching back, often from bench press, dips or throwing. Rehab unloads first, then adds slow eccentric strengthening.
AC joint sprain
Pain on top of the shoulder that hurts when reaching across the body, usually after a fall on court or push-ups. Rehab combines kinesio taping with protected loading.
Scapular dyskinesis
The shoulder blade sticks out, with clicking and early fatigue from a weak lower trapezius and serratus anterior. Rehab trains the shoulder blade stabilisers.
Partial rotator cuff tear
Sudden weakness and an inability to hold the arm out, often after a fall or a very heavy lift. Needs specialist evaluation, with conservative rehab where suitable.
Warning signs you should not play through
How Complete Care Treats Sports Shoulder Pain
Treatment starts with finding the real cause, not just the sore spot. At our shoulder pain treatment in Ahmedabad programme, the first 45-minute assessment includes:
- Impingement tests (Neer and Hawkins-Kennedy) to check for tendon pinching
- Empty can and full can tests to check supraspinatus strength
- Scapular assistance tests to see if supporting the shoulder blade eases the pain
- A movement check of your serve, throw or lifting form where relevant
Your plan is then built in phases. Most patients need 6 to 12 sessions of 45 to 60 minutes, depending on the condition.
The Four-Phase Shoulder Rehab Plan
Calm it down (weeks 1 to 3)
Reduce pain and irritation with Class IV laser therapy, IFT or TENS, load modification and Kinesio taping.
Restore movement (weeks 2 to 5)
Regain full, pain-free range using manual therapy joint glides, posterior capsule stretching and dry needling for trigger points.
Build strength (weeks 4 to 10)
Develop rotator cuff and shoulder blade endurance with band external rotations, prone Y and T raises, and scapular wall slides.
Return to sport (weeks 8 to 12)
Rebuild safe overhead loading through graded serves and smashes, gym programme re-entry and a technique review.
Our sports physiotherapy team adds racket, throwing and agility drills in the final phase. If your shoulder feels stiff in every direction rather than painful in one arc, read about frozen shoulder treatment instead.
To understand the small stabilisers that protect the joint, see our guide on why small shoulder muscles are key to pain-free movement. If the pain is worst at night, our article on shoulder pain while sleeping covers positioning in detail.
Do's and Don'ts for a Painful Shoulder
Protect the Joint Between Sessions
Warm up for 8 to 10 minutes
Use a resistance band before play or lifting. Do not jump straight into smashes, throws or heavy presses.
Increase load in small steps
Add playing hours or weight gradually each week rather than doubling your game time in one week.
Train the upper back and cuff
Work these at least twice a week instead of training only chest and front shoulders.
Sleep on the other side
Support the painful arm with a pillow. Avoid sleeping with that arm stretched overhead.
Get a technique check
Have your grip, serve or bar path reviewed rather than copying random stretches from social media reels.
See a physiotherapist early
Book if pain lasts more than 3 to 5 days. Do not depend only on painkillers to keep playing.
A 5-Drill Shoulder Warm-Up Before You Play
These drills take under 10 minutes and prepare the small stabilising muscles that protect the joint. Stop any drill that causes sharp pain.
- Band pull-aparts: 15 reps, squeezing the shoulder blades together.
- Band external rotations: 12 reps each side, elbow tucked at your waist.
- Scapular wall slides: 10 slow reps, forearms sliding up the wall.
- Thoracic open-book rotations: 8 reps each side, lying on your side.
- Arm circles, small to big: 20 seconds forward and 20 seconds backward.
When to See a Physiotherapist
Book an assessment if your shoulder pain lasts more than 3 to 5 days, disturbs your sleep, stops you lifting your arm overhead, or makes carrying things feel weak.
